Medical Licensing

NLST, LLC and its operators are Certified Sound Therapists and Technicians and NOT a licensed medical provider and does not diagnose nor prescribe frequencies, medications or supplements and any advice is not to be replaced with the advice of a licensed medical practitioner.

Client responsibility

It is the responsibility of the client to disclose any underlying physical condition or illness that would prevent them from therapy. NLST, LLC will not provide treatment if we are made aware of adverse conditions.

Consult your physician before using any auditory and / or vibratory input devices if any of the following apply:

• You have an implanted electrical device
• Severe panic disorder
• Epilepsy
• Recent head injury or surgery
• Severe autism spectrum disorder
• Severe PTSD

Consult your physician before using any PEMF device if any of the following apply:

• You have an implanted electrical device
• You are pregnant
• Recent head injury or surgery
• Organ transplant patient

Prior to treatment

Please remove all Electronic or battery-operated devices, keys, wallets, cards with magnetic strips such as credit cards and hotel keys, your watch, hearing aids, cellphones, laptops, etc.

Next Level Sound Therapy, LLC and our employees are not responsible for damage to technology that has not been removed prior to your session.

Consent

Payment for services is consent for therapy to be performed by NLST, LLC and its staff. You give permission and authority to provide therapy in ways that you have determined beneficial for you. You feel adequately informed to do so and you accept all liability for your health and wellness choices and hereby hold NLST, LLC and its representatives harmless.

IF UNDER 18 YEARS OF AGE YOU WILL NEED THE CONSENT OF A PARENT OR PHYSICIAN.

You will sign a copy of these terms on your first visit.
